They are at their office and Sean McNamara has Alejandro and Perez as his patients. The problem, however, is that Perez speaks Spanish and Sean cannot understand it. So he goes up to Christian Troy, who is in the middle of a butt implant surgery and asks him to talk out with Perez as Troy knows Spanish and Sean would do the implant, which by the way Troy has done the wrong way.
Troy talks out to the brothers and settles the amount for the surgical procedure they want to go through at $300,000, a sum about which Sean has no idea till now.

Before going home Troy visits a bar where he hooks up with Kimber, a blonde model. The couple head to Troy’s house to have sex. Even Sean is having sex at home with his wife Julia, but their sex is boring as Julia has her mind clouded with grocery and home affairs, and Sean is thinking about chucking out the gardener, Julio.

The following morning, Kimber lies to Troy that she is perfect at twenty one. Troy chips in with the fact that Kimber was twenty six years old and should have some work done upon her. At Sean Mc Namara’ home, Julia is confirming Sean’s views on her breasts, and the husband gives a mechanical answer rather then his personal opinion. He is not happy with Annie also, who now is getting a gerbil for herself. Sean’s children, Annie and Matt have a chat on alligators and are displeased over the fact that their father never pays attention to their talks.

Matt, instead of going to the school visits Troy at his boat the BOATOX and asks his opinion regarding being circumcised. Troy informs him that his father’s consent would be necessary for the procedure.

There is a mother of a burn victim pleading to Sean for a surgery at his office but he declines because he claims he is too busy.

Mc Namara and Troy prepare for Perez’s operation and have a chat about Sean’s son Matt. Sean is firm that there would never be any surgery in his family. Sean later learns that $300,000 was charged for the surgery and questions Troy about it, telling him the money was earned through drugs operations. Troy tells him that he doesn’t give a damn.

Troy, after some time, meets Julia and talks to her about life in general, while in the office Sean is talking out with Matt on circumcision. Sean ultimately concludes that his son had to see a specialist or a psychologist and would have to find one for him.
Then Troy learns that Perez had fiddled with his boss’s six year old daughter and that is why he was on move. Troy feels sick to hear that.

Kimber, Troy and Liz have a confrontation and when Liz is questioned by Sean Mc Namara she tells him curtly that Troy was rubbishing his reputation and name. Sean then sees Troy and explains to him that he was leaving him and would work from somewhere else.

Julia is watching a recent tape of Sean, her and Troy while Troy comes to offer some explanation to Julia. Julia is in tears as she narrates him that she was now not allowed for car pooling as she had flushed the gerbil into the toilet. As he is comforting her, he asks Julia to show her breasts, and as he is examining the breasts, which to him seems perfect, Annie comes in and Troy leaves reminding Julia that they would have some time together at his place to which Julia instantly agrees.

After firing off the gardener Julio, Sean comes home and has an argument with Julia so he exits.

The next day, Troy tells Sean that he would have to buy his share as well or he will grab it from him. Celia is visited by Christian and commits to hand over ten injections of Botox. At breakfast, Julia drains out Annie’s gebril in toilet.

In the hospital Sean is told by Alejandro that Perez desired for a thinner waist. The boss of Perez, Escobar, who is husband of Celia, abducts Troy who is injecting her botox and pressurizes him into telling the whereabouts of Perez. Sean has just started the surgery as Alejandro takes out a gun and wakes Perez to tell him that he has a baby girl too and Perez would never be able to touch her. On the other hand, Escobar who is looking for Perez injects botox in Troy’s face and crotch. Sean arrives and informs them what had just happened.

Perez is dead by the time Escobar and Troy come in too see him. Escobar accompanied by Alejandro and Liz exits to visit the supermarket and purchase hams. They come back; wrap the ham around Perez for a meal of the alligators.

Troy concludes in the end that they should have a psychologist to check on their clients before operating them but Sean comments that approximately twenty percent of their clients would be just pro bono.
